The topographical pattern of the esophageal varices was assessed in relation to local and systemic characteristics. No correlation was found between esophageal varices and either their diameter, the number of varices, or the presence of portal or gastric venous collaterals. Esophageal varices were more frequent (p less than 0.01) in patients with a history of variceal bleeding than in patients without this complication (56% vs 26%). The esophageal varices were always found in the lower third of the esophagus. In patients with and without variceal bleeding, esophageal varices had the same pH and were more often refluxing when pH was raised than when it was low (p less than 0.001). In patients with variceal bleeding, higher pH values were observed at the site of the varices (p less than 0.01). These results suggest that (1) esophageal varices are more frequent in cirrhotic patients with a history of bleeding and (2) esophageal varices in cirrhotic patients without bleeding might be caused by portal or gastric venous collaterals 08929e5ed8
